One in three people infected with SARS-CoV-2 and who have not been vaccinated no longer have detectable antibodies one year after infection, according to a seroprevalence analysis conducted on more than 1,000 people between the ages of 43 and 72 by the Barcelona Institute for Global Health (ISGlobal) and the German Trias i Pujol Institute (IGTP).

As reported by ISGlobal this Friday, the study, which also had the support of the Daniel Bravo Andreu Private Foundation (Fpdba), underlines the need to be vaccinated despite having been infected and confirms that hybrid immunity (vaccination plus infection) is greater and more durable. This work has just been published in the journal ‘BMC Medicine’.

According to the researchers, both infection and vaccination against SARS-CoV-2 contribute to building a population’s immunity against the virus, which is important information for deciding when and to whom to apply booster vaccines.

Therefore, the easiest strategy to assess said immunity is to carry out seroepidemiological studies (that is, to quantify virus-specific antibodies in a certain population group), although immunity against a pathogen goes beyond antibodies.

In this regard, ISGlobal researcher and senior co-author of the study, Manolis Kogevinas, together with fellow ISGlobal researcher Carlota Dobaño, explained that “most of the serological studies carried out after vaccination against covid focused on specific groups such as health personnel, they did not distinguish between people with or without previous infection, or they did not have clinical and immunological data of said infection”.


In this study, the research team carried out a second measurement in a Covicat-GCAT population cohort from Catalonia (GCAT | Genomes for Life Project) six months after the start of vaccination (the first was just after the first confinement) to follow up of the level and type of antibodies against various viral antigens.

They also used information obtained from a questionnaire and health records to identify factors that potentially determine the magnitude and duration of response in unvaccinated, vaccinated, or vaccinated and infected persons. 1,076 people were included in the analysis, between 43 and 72 years of age.


Their results showed that 36% of infected but unvaccinated people no longer had antibodies one year after infection, especially in people over 60 years of age and smokers.

In addition, in previously infected people, vaccination induced significantly higher antibody levels than in previously uninfected people. These levels were strongly associated with the magnitude of the response during infection.

In this regard, the first author of the study, Marianna Karachaliou, together with Gemma Moncunill, highlighted that “our data underscore the importance of vaccinating people, even if they have been previously infected, and confirm that hybrid immunity is superior and more durable. This means that people who have been vaccinated but have not passed the infection would need a booster sooner than those who have passed it.”

As for the vaccines, the researchers found that Moderna had more antibodies.
